Fix pack/unpack for UCI.Indication 

Fix PUCCH Format2/3/4 value to be indexed at 0, not 2.
This is according to Table 3-66 of SCF222.10.02

Add utility functions related to UCI.indication
Extra utility function to calculate allocated memory added.
